Patient Data & HIPAA Disclosure Notice

This Notice describes how the coordinated network of participating licensed dental providers may use and disclose your protected health information (PHI) and how you can obtain access to that information.

Participating providers are independently responsible for HIPAA compliance under their own credentials. During a call to our dispatch line, only the minimum information necessary to route your request is collected; PHI generated during your clinical evaluation is maintained by the provider who delivers care.

You have rights regarding your PHI including the right to request access, amendment, and an accounting of disclosures. Contact the participating provider directly to exercise these rights.
